Tyde Medical Center LLC understands that your health information is personal. We are committed to protecting it. This Notice applies to all records of your care generated by Tyde Medical Center LLC, whether made by our personnel or your personal doctor or another healthcare provider.

You have the right to:

- Request amendments to your PHI.

- Request confidential communications.
- Obtain a paper copy of this Notice at any time.
Complaints
If you believe your privacy rights have been violated, you may file a complaint with our Privacy Officer using the contact information above or with the U.S. Department of Health and Human Services, Office for Civil Rights. We will not retaliate against you for filing a complaint.
