Our Pledge Regarding Protected Health Information

Tyde Medical Center LLC understands that your health information is personal. We are committed to protecting it. This Notice applies to all records of your care generated by Tyde Medical Center LLC, whether made by our personnel or your personal doctor or another healthcare provider.

We are required by law to:

  • Make sure your protected health information (PHI) that identifies you is kept private.
  • Notify you about how we protect your PHI.
  • Explain how, when, and why we use and disclose your PHI.
  • Follow the terms of this Notice currently in effect.

We reserve the right to change this Notice and make the new provisions effective for all PHI we maintain. If we make material changes, we will:

  • Post the revised Notice on our website.
  • Make copies available upon request.

 

How We May Use and Disclose Your PHI Without Written Authorization

For Treatment

We may use PHI to provide, coordinate, or manage your medical treatment or services, and disclose PHI to doctors, nurses, pharmacies, laboratories, or others involved in your care.

For Payment

We may use and disclose PHI so the treatment and services you receive can be billed to and payment collected from you, an insurance company, or a third party.

For Healthcare Operations

We may use and disclose PHI for operations such as quality assessment, training, case management, and business planning.

 

Your Rights Regarding Your PHI

You have the right to:

  • Inspect and copy your PHI.
  • Request amendments to your PHI.
  • Receive an accounting of certain disclosures.
  • Request restrictions on certain uses and disclosures.
  • Request confidential communications.
  • Obtain a paper copy of this Notice at any time.

 

Complaints

If you believe your privacy rights have been violated, you may file a complaint with our Privacy Officer using the contact information above or with the U.S. Department of Health and Human Services, Office for Civil Rights. We will not retaliate against you for filing a complaint.

 

No signature is required to view this Notice. This page is for informational purposes only and applies to all patients of Tyde Medical Center LLC.
